Strength training: the health dose for every age and why it matters

Short answer

Strength (resistance) training is not only for athletes and those who want to build muscle; it is a fundamental component of health at every age. Alongside aerobic activity, the World Health Organization recommends strength exercise working the major muscle groups on at least 2 days per week. Regular strength work is associated with lower overall mortality and lower risk of cardiovascular disease, diabetes, and cancer, preserves bone density and muscle mass, reduces falls in older age, and is one of the strongest tools in preventing sports injuries.

It is safe: with correct technique and gradual progression it is suitable for healthy adults of any age. You don't need heavy loads or a gym to start — you can begin with your own body weight, resistance bands, or light weights.

How much? (World Health Organization, current recommendation)

  • Adults (18–64): 150–300 min of moderate or 75–150 min of vigorous-intensity aerobic activity per week + at least 2 days per week of moderate or greater intensity strengthening working all major muscle groups.
  • Aged 65 and over: In addition to these, multicomponent activity including balance and strength on at least 3 days per week — to maintain function and prevent falls.

Effects on health (the evidence)

  • Mortality and chronic disease: In analyses pooling large cohort studies, regular strength activity has been found associated with roughly 10–17% lower risk of overall mortality, cardiovascular disease, cancer, and diabetes. An interesting finding: the benefit does not require very long durations per week — about 30–60 minutes per week of resistance exercise is associated with the highest benefit.
  • Bone: Resistance training can preserve and slightly increase spine and hip bone density; it is valuable in managing osteoporosis risk.
  • Muscle (sarcopenia): Resistance training is the most effective way to preserve the muscle mass and strength lost with age; this is the key to independence.
  • Metabolic health: A positive contribution to insulin sensitivity and blood sugar control.
  • Injury prevention: In prevention programs applied to specific sports populations, strength work can reduce injury risk (markedly in some meta-analyses); the effect varies by sport and program.

In older age: strength alone is not enough, together with balance

The evidence on fall prevention is clear: the most effective approach is balance and functional exercises; strength reduces falls when added to them. Working only on strength while neglecting balance does not lower fall risk as much as expected. That is why the recommendation for those over 65 is "strength + balance."

A safe start

  • 2 days per week, major muscle groups (legs, hips, back, chest, abdomen, shoulders).
  • Technique first, weight second. The type of equipment (free weights, machines, bands, body weight) does not determine the outcome; effort and gradual progression do.
  • Meaningful gains are achieved even with light loads at the start; increase the load over time, in small steps.
  • Those with a chronic illness or who have been inactive for a long time should consult their physician before starting a program.

For clinicians

Mortality/NCD data are predominantly observational (association, not causation); the evidence on muscle mass, bone density, and fall prevention is RCT-based and stronger — this distinction should be shared with the patient. The J-shaped dose-response in Momma 2022 (optimum around ~30–60 min per week) is useful for giving the patient a "little but regular" message. In older adults the prescription must include a balance/functional component (Cochrane/Sherrington). Current position statements consider resistance training safe in healthy adults of all ages.
